Instaviz lets you brainstorm ideas quickly, rather than bogging you
down with exact placement of nodes on the screen. I think it would be
difficult for a user to control actual placement effectively on a
small screen, and also the user interface would be more clunky since
we'd have to account for drags to link nodes, scrolls/zooms, sketches
to create nodes and then drags to reposition nodes.
For 1.6 or 2.0, I'm hoping to implement "re-ordering" nodes. While
this won't actually allow you to reposition the node anywhere, you
would be able to swap the order of two nodes e.g. looking at the
diagram below, swapping 1 and 2 or swapping 7 and 8.

You can use the whole screen to draw, and it doesn't have to be
particularly accurate for Instaviz to recognize it. In 1.5, you have
to select a node before you can change its shape, so it's easier to
draw new shapes, even overlapping shapes, without destroying older
nodes.
I do still have a problem with diamond recognition and hope to fix
this in the next version.

There is a known problem where if you sketch objects to the top or
left while the diagram is small, then when the diagram grows bigger,
the objects are "off-screen". If this is your problem, you can
sometimes still connect nodes if you zoom in by pinching with your
fingers.
I hope to fix this for 1.6 or 2.0.

Were you trying for this:

I made that fairly quickly in Instaviz.
Nevertheless, I appreciate your issues and ask for your patience.
Instaviz has been out for a year and I've updated it 5 times already
-- I actively keep track of mailing list emails, some forums pages,
google results and app store reviews and regularly compile bugs and
new features to work on. So you can be sure your issues will be worked
on. See for example:

If you still want a refund, the usual approach is through Apple
directly. Go to http://www.apple.com/support/itunes/#app_store, click
on the Email Us button and detail your request for a refund.
